What Is Delayed Ejaculation? Everything Men Need to Know

Delayed Ejaculation (DE) affects at least 5% to 10% of males. This is a condition where prolonged sexual stimulation has to be administered to a man to achieve orgasm, or someone who cannot ejaculate at all. Before we embark on telling men what they need to know, here’s what they don’t know –

While DE is highly misunderstood, it is a recognized and documented clinical condition, which is often confused with the lack of physical arousal. 

For better clarity and information, here’s what men need to know, from defining DE to diagnosis and treatment. 

What is Delayed Ejaculation? 

When you need extensive or prolonged stimulation or take a long time to achieve sexual climax and semen release, it is known as delayed ejaculation. In many cases, there is no ejaculation at all. If DE happens when you are masturbating or having sexual intercourse with a partner, it can become highly frustrating. While there is no real normal time limit as to how long an ejaculation could take, it can cause stress and become a prolonged and chronic issue. 

Symptoms of Delayed Ejaculation

The symptoms of DE are pretty much clear. You just have trouble ejaculating. Typically, it only takes a few minutes of stimulation for men to ejaculate. But when it’s delayed, say you take 30 minutes or longer, then it becomes frustrating. Other symptoms include:

  • Getting tired physically
  • Experiencing dyspareunia or pain during intercourse
  • Anxiety
  • Feeling undesirable
  • Stress
  • Feeling inadequate 

There is no doubt that DE can affect you and your sexual partner emotionally and mentally.

Causes Of Delayed Ejaculation 

DE can be caused due to mental, emotional and physical stress. 

1. Physical Causes

  • If you’ve had a stroke, multiple sclerosis, or a spinal cord injury, these affect the nervous system and lead to DE
  • Any nerve damage occurring as a complication of surgery or diabetes
  • Ejaculatory duct obstruction or a blockage in your penis
  • Antibiotics, antidepressants, blood pressure medications, antihypertensives and some prescription medications or alpha blockers
  • Using recreational drugs or overconsumption of alcohol
  • Natural changes to your testosterone levels due to aging, low libido and reduced sensitivity in your penis

2. Psychological Causes 

  • Guilt during sexual intercourse – this is primarily if you are heavily religious or have had a unique upbringing
  • Uncomfortable or angry with your sexual partner
  • Fear of STIs, pregnancy or childbirth (tokophobia); Also fear of hurting your partner emotionally and physically 
  • Anxiety about performing sexually
  • Addiction to sex or pornography 

In some cases, the physical causes of DE may be ruled out if you are able to orgasm easily by yourself and yet have difficulty ejaculating with a sexual partner. There are times when your partner isn’t able to duplicate a technique that best suits you, and simultaneously, you are unable to ask your partner. These cases are not diagnosed as DE. 

3. Aging

As they get older, men experience delayed ejaculation. It just so happens that they need more stimulation and time to ejaculate. The volume of semen and force of ejaculation may also be decreased. While the age to stop ejaculating is not benchmarked, the testosterone production starts to deplete when you turn 30. This hormone is responsible for enhancing your sex drive and producing essential sperm. Some men in their 40s may have delayed ejaculation, too. But then there are others who ejaculate even when they are 70 or older. DE can affect some men their entire lives, and for others, it happens rarely. But it does begin occurring regularly as they get older. 

Diagnosis 

Your ejaculation patterns, sexual habits, and medical history are collected by the health evaluator. There could be a checklist of questions, such as when you first noticed DE and whether it happens only sometimes or often. Other questions could include:

  • Can you ejaculate during masturbation?
  • How long does it take if you ejaculate?
  • Can you ejaculate during sexual intercourse?
  • Have you had a penile fracture, erectile dysfunction or a penile disorder in the past?
  • Are you taking a prescription medication right now?
  • Have you had any changes or new sexual desires?
  • Do you drink excessive alcohol or use recreational drugs?

Feeling awkward during this questioning is normal, but it is extremely important that these be answered truthfully for an appropriate diagnosis. 

Physical Exam

A physical examination may be performed to ascertain the degree of disorder. Certain tests may also be conducted to rule out other conditions. Blood tests, urinalysis or a pee test, and a semen analysis, especially if you are trying to have a biological child, may be ordered. 

Treatment for Delayed Ejaculation

While there isn’t a single treatment modality, there are several ways to treat DE. Some could involve medications; others would require nonprescription drugs. In many cases, men are advised to avoid alcohol and recreational drug use. A sex therapist or a psychotherapist may also be recommended for counselling. If DE is because of your partner, then both of you might require counselling.
